Any Guild member incurring expense on behalf of the guild should print and complete this form. Completed forms must be submitted to the Guild Treasurer for reimbursement from Guild funds.

Name Tag Pattern and Instructions
Members are encouraged to wear a name tag to general Guild meetings. The pattern included here is a small version of the classic Laurel Wreath block, which is the basis for our guild logo.
